UART Adapter V2 - Issues

Earlier this week I plugged in my UART Adapter V2 to both my vehicle's OBD-II port and my Arduino Mega. Everything appeared to be working correctly (I saw the blue LED on the Adapter light up) and the Arduino powered up.

Fast forward to yesterday afternoon when I plugged everything in again, there was no blue LED on the Adapter. I double checked the pin configuration on the Arduino (routed through the Freematics 3.2" TFT shield), but no luck.

When I went to measure the voltage output of the Adapter with my multimeter, it appears to be outputting at 2.5V - not enough to power the Arduino fully. I've also confirmed the Arduino is still functional by plugging it into my computer via USB.

Can anyone offer any advice? Is it possible I'm working with a defective unit?

What's the output voltage without load?
